Office Policies
I look forward to seeing you on your scheduled appointment date. My office may contact you a few days prior to your appointment to confirm the appointment date and time, pre-register standard information and remind you of the upcoming appointment. When checking-in, it may be necessary to obtain copies of insurance ID card(s) in order to bill your insurance company.

My office is closed from 12:00pm to 1:00pm for lunch.
Appointments & Cancellations
In the event that you need to cancel your appointment, kindly provide 24 hours notice. Your consideration in this matter will allow us to accommodate other patients who could benefit from an appointment. Please extend us this courtesy so that other patients may be seen.
Emergencies
Our practice provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week. Realizing that you can have an emergency arise, we will do our best to respond to your problem promptly. In the event of a severe situation go immediately to the hospital. All calls will go directly to an answering service and the physician on call will be notified. Please restrict your calls to urgent medical questions only. Call 972-981-7711.
Payment
All co-payments and deductibles are due at the time of service. This includes payment for any surgical procedures, which is usually taken care of in advance.
Most insurance plans require you to pay a predetermined amount (your "deductible") before insurance will pay. Our technology allows us to view your remaining deductible and help you understand what you will owe for your visit so we can collect the amount due at the time of your visit. For new patients who have not yet met their deductible, we may collect up to $100.00; for established patients, we may collect up to $75.00. This payment will be applied to your visit. When your insurance completes processing of your health insurance claim, you may be responsible for an additional amount depending on our contract with your insurer.
NOTE: If you use our Card on File process, you will not be required to pay a deposit at the time of your visit.
My office accepts cash, personal checks as well as the following credit cards:

Prescriptions
Prescriptions and/or refill requests should be obtained at the time of your appointment. If you do not have an appointment, prescription refills will be processed within 48 hours. For prescription refills, we ask that you call your pharmacy and ask them to FAX the request to our office. This will facilitate your request in a timelier manner. Once the doctor has authorized the refill, it will be forwarded to your pharmacy. If a new prescription is required, please allow 3 business days for mailing. Indicate whether you need a 30 -day or a 90 -day prescription. If there is a problem with the request, you will be notified. If you have not been seen by your doctor within the last twelve months, no prescription refills will be issued. It will be necessary for you to schedule an appointment in order for your prescription to be refilled.
